The following hazards were observed: 1. Two of the toy shelves in the school age room were observed to be damaged. One of the shelves had a large hole in the top of it and the surfacing on the other shelf was in despair as the adhesive material is chipping and has separated from the frame exposing the particle board underneath. There was another shelf that had a buildup of dust and grime and in need of being cleaned. 2. Seven outlets in the Pre-K 1 room did not have childproof covers.
